What Actually Drives the Cost of Private Health Insurance
People ask me for a price before I know anything about them. I cannot give an honest one that way, and anyone who does is guessing. Here is what the premium is actually built from, so you know what you are controlling.
1. Age
Older applicants pay more on every kind of plan. Marketplace plans are limited to a 3-to-1 ratio between oldest and youngest adults. Private plans use their own age bands. Either way, the price you get at 30 is not the price at 50.
2. Health history
This is the big one on private plans and the thing that does not exist on the marketplace. Private carriers ask about conditions, medications, hospitalizations and tobacco. A clean history gets the best rate. A controlled condition may get a higher rate or an exclusion. Some histories are declined, and then the marketplace is the answer.
3. Zip code
Medical costs vary by county, so the same plan costs different amounts in Tampa, Atlanta and Dallas. Carriers also file different rates by state.
4. Deductible and out-of-pocket maximum
The more you agree to pay before the plan pays, the lower the premium. The trick is picking a deductible you could actually cover in a bad year. A plan with a deductible you cannot afford is not really coverage.
5. Network
A broad national PPO costs more than a narrow local HMO. For people who travel, run a business, or want to keep a specific doctor, the PPO is worth it. For someone who never leaves the county and is happy with the network hospital, the narrower plan can be a fair trade.
6. Who is on the policy
Spouse and kids each add to the premium. Child rates are usually low. Sometimes it is cheaper to split a family across two plans, and I check that every time.
What you can control
Deductible, network, who is on the plan, and when you apply. Your age and zip code are what they are. Your health history is the reason to apply while you are healthy rather than waiting.
Getting a real number
Fifteen minutes on the phone. Zip code, ages, medications, doctors you want to keep. You get marketplace and private side by side with the network checked. No fee, because the carrier pays me either way.
Frequently asked questions
Why won't you just post prices?
Because an honest price depends on your age, zip code, health and the plan design, and a number posted for a hypothetical person would be wrong for you. Quotes take fifteen minutes and are free.
Is private always cheaper than the marketplace?
No. For households that qualify for a subsidy, the marketplace is often cheaper. Private plans usually win for healthy people above the subsidy range.
Does the premium go up every year?
Rates can be adjusted at renewal, and age bands move as you get older. Some private plans offer a rate guarantee for a set period. I show you the renewal history and any guarantee before you commit.
Is there a fee to work with you?
No. The carrier pays the broker. You pay the same premium whether you buy through me or directly, and through me you get a human to call when there is a claim problem.
